- The ZMed Clinic Giveaways through the 12 Days of Treatments and similar programs are extended in the discretion of the management of ZMed Clinic, and is made on behalf of the ZMed Clinic family of companies, including Clinic Service Providers, PA, that provides medical care to patients at locations managed by ZMed Clinic. Any medical decisions are made by consultation with the licensed medical staff of Clinic Service Providers, PA, and any treatment, prescription, of medical devices will not be used against the medical advice of the its clinicians. ZMed Clinic and Clinic Service Providers, PA expressly reserve the right to refuse any treatment, for medical concern or otherwise.
- Acceptance of any benefits constitutes your acceptance to be bound by Terms and Conditions without limitation, qualification or change. Additionally, you must agree and execute customary procedure consent forms. If you do not accept all the Terms and Conditions of this Agreement and if you do not also agree to the language of applicable procedure consent forms, you must decline or immediately discontinue treatment from ZMed Clinic.
- Cosmetic and elective procedures are only performed at your request. Individual response to lasers, fillers, toxins, and other cosmetic procedures vary. Accordingly, results from these procedures may vary in degree, timing, and the number of treatments required.
- Dysport is a proprietary form of botulinum toxin A, a neurotoxin in widespread use to lighten the appearance of facial lines and wrinkles. Dysport is similar to Botox in substance and application, but Dysport is manufactured by Galderma, whereas Botox is manufactured by Allergan, who has perfected trademark protection for the brand Botox. The name Botox herein is only used for comparison purpose, not to indicate that Allergan’s product is available or offered.
